Standards, Operating Procedures, and Best Practices for Diabetes Prevention Webinar

Health centers are required to track diabetes measures and its action planning using root cause analysis. Health centers will have to identify three actions to improve or sustain performance on the measure and input their action plans in the HRSA Electronic Handbooks (EHBs). The CDC has developed the National Diabetes Prevention Program (NDPP) to assist in diabetes prevention or delay of type 2 diabetes. This webinar will present information on best practices in diabetes care, patient engagement, and self management to reduce the risk of type 2 diabetes and improve overall health.

Participants will learn to:

  • Deliver CDC-recognized lifestyle change programs
  • Ensure quality and adherence to proven standards of care
  • Implement effective training programs
  • Increase referrals to and participation in lifestyle change programs​​​​​​​
