APHCA provides programs and services to assist community health centers to learn how to redesign all aspects of their health care system through the use of the Planned Care Model and the Improvement Model, national treatment guidelines and core performance measures and patient outcomes data. APHCA members can achieve results organization-wide as opposed to within just one department or area. The strategies used by APHCA assumes that health centers are not bound by the current system, aspire to constantly improve and that they desire a system that is efficient, effective and satisfying for both patient and staff.

Delivery System Design
- Assure the delivery of effective, efficient clinical care and self-management support.
- Define roles and distribute tasks among team members.

Decision Support
- Promote clinical care that is consistent with scientific evidence and patient preferences.

- Use proven provider education methods.
- Share evidence-based guidelines and information with patients to encourage their participation
Clinical Information Systems
- Organize patient and population data to facilitate efficient and effective care.
- Provide timely reminders for providers and patients.
- Identify relevant subpopulations for proactive care.
- Facilitate individual patient care planning.
- Share information with patients and providers to coordinate care.
- Monitor performance of practice team and care system.
