DESCRIPTION:Venue: APHCA Training Center \nDescription: The April CLIMB Committee meeting will build upon the foundation outlined in February.  The CLIMB group will work to reconcile data linking diabetes\, depression\, and social determinants of health and then utilize that data in breakout groups to create actionable PDSAs that will focus on improving clinical quality measures in those areas.  In addition\, the group will examine workflows in relation to these measures as well as closing the loop on patient referrals.  Finally\, the CLIMB group will begin discussions around patient engagement and targeted outreach campaigns.
