DESCRIPTION:Venue: APHCA Training Center \nPresenter: APHCA Quality Team \nAudience: CLIMB Members \nDescription: The CLIMB workgroup is a collaborative community of clinical and quality staff from various community health centers. The group aims to promote partnerships and support within the network while achieving high-performance levels for all member organizations. They work towards addressing and reducing performance variances within the network. The CLIMB group functions as a “working committee” to develop standardized best practices\, identify workflow process improvements\, and guide the network in developing quality improvement goals and objectives. The group’s focus areas for 2024 include Risk Stratification\, Care Coordination/Care Management (RPM)\, Telehealth\, Data Analytics\, and various Quality improvement initiatives. Each group session discusses these topics and aims to fully integrate them into patient-centered care to move the Network towards a value-based care platform.
