The June CLIMB Committee meeting will focus on collecting social determinants of health and best practices for patient engagement platforms and how these tools can be integrated with current data collection procedures. The CLIMB group will examine workflows in relation to these measures and see how well we are closing the loop on patient referrals and/or other identified gaps in care.
